1.Click the Settings tab.

2.Click Accounts from the accordion menu on left side of the screen.

3.Click the Custom Fields link.

4.Click the Add New Custom Field button. The Add New Custom Field screen will display in an editable format. The Question field is where the name of the Custom Field is entered along with the instructions for the response desired and the category.

5.The Response field has several options and contingencies:

a. The Text response option allows the respondent to enter a detailed response. The height and width of the text box, the number of text rows, max numbers of characters, and the option of a Default Value are all customizable features available under Text Options.

b.The Number response option allows the respondent to enter a numeric response. The height and width of the number box, decimal place options, minimum and maximum value options, out of range message, and the option of a Default Value are all customizable features available under Number Options.

c.The Date response option allows the respondent to enter a date response. The height and width of the date box, minimum and maximum value options, out of range message, format and the option of a Default Value are all customizable features available under Date Options.

d.The Yes/No response option allows the respondent to submit a simple Yes/No response. The only additional option is the Default Value under Yes/No Options.

e.The Selection List response option allows the respondent to select a response from a list of options. List options, Selection type, Format, height and width, layout, repeat direction, and number of columns are all customizable features available under Selection Options.

f.The Link response, the Sub-Group response, and the Custom response are all advanced responses that need to be handled by an Administrator.

6.The Advanced Options allow the user to customize the layout of the Custom Field on the screen, determine the font size and color, add a prefix or suffix to the response, and control who can see, and what level interaction others have, with this this information.

7.Click the Save button win finished.
